Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to join our Owensboro, KY team. You will examine and treat patients to improve mobility and independence in daily living within a home-based care setting.

Our programs include Balance and Fall Prevention, Spine Safety, and Joint Replacement care. You will deliver skilled therapy under physician orders and coordinate with the Branch Director and care team to support recovery at home.

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Provide physical therapy to patients to improve mobility and daily living skills.
• Utilize Balance and Fall Prevention, Spine Safety, and Joint Replacement program protocols.
• Administer skilled care under physician orders and plan of care.
• Educate patients and families on home-based therapy and exercises.
• Collaborate with the health care team and Branch Director.
