Job summary

TEEMA Group is seeking a Home Health Physical Therapist to deliver physician-prescribed, goal-driven care in patients’ homes, focusing on improving mobility and quality of life.

You will conduct assessments, design individualized plans of care, supervise therapy assistants, educate patients and families, and coordinate with physicians. Travel within a 60-mile radius with a caseload of about six visits per day, Monday to Friday, 8:00 AM – 5:00 PM.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.
• Current, unencumbered Physical Therapist license in good standing with the state Board of practice.
• Valid driver’s license and current automobile insurance.

Responsibilities

• Perform initial and ongoing comprehensive assessments, including OASIS assessments at designated time points.
• Maintain a daily caseload of approximately six patient visits per day within a 60-mile travel radius.
• Participate in plan of care development and implementation with attending physicians.
• Deliver skilled restorative treatments to reduce pain and restore or maintain physical function.
• Instruct, establish, and supervise home exercise programs and provide home safety instruction.
• Direct and supervise Physical Therapist Assistants, CNAs, and HHAs.
• Participate in team meetings to coordinate care and staff education.
• Provide clinical instruction and guidance to patients and families.
