Job summary

San Ysidro Health is seeking a licensed Physical Therapist to join the PACE interdisciplinary team in California. You will design and implement a comprehensive PT program for PACE patients, assess abilities, and develop individualized care plans to maximize independence in the community.

You will supervise physical therapy assistants, ensure adherence to plans, and conduct reassessments. Travel to patient sites will be required, and you will stay current with standards and regulations affecting

Qualifications

• Licensed Physical Therapist in CA.
• CA driver’s license and reliable transportation.
• BLS/CPR certification.
• Experience with geriatric population preferred.
• Bilingual English & Spanish preferred.

Responsibilities

• Develop and implement PT program for patients enrolled in PACE and access day services.
• Perform assessments of patients to determine abilities.
• Develop individualized plan of care to decrease disability and maximize function.
• Supervise therapy assistants and ensure plans are followed.
• Provide therapies to relieve pain and restore function using modalities.
• Record and sign treatments in health records; maintain documentation.
• Travel to home, hospital, and other sites as needed.
• Stay abreast of regulations affecting PACE, ADHC, CMS, DHCS.
