Job summary

San Ysidro Health is seeking a Registered Nurse to join the SD PACE program as an interdisciplinary team member. You will provide direct and indirect nursing care, apply advanced nursing knowledge, and collaborate with multiple disciplines to ensure safety and high-quality care for participants.

Responsibilities include comprehensive assessments, care planning with families, IV therapy, patient education, and coordinating with physicians and support staff.

Qualifications

• Graduate of an accredited school of nursing.
• California RN licensure required.
• BLS certification required.
• Experience with geriatric or PACE populations is preferred.

Responsibilities

• Conduct comprehensive nursing assessments and data collection.
• Develop and update care plans in collaboration with the IDT.
• Provide direct and indirect nursing care and patient/family education.
• Coordinate care with physicians and other health professionals; monitor and document patient status.
