Job summary

St. Paul’s PACE Corp (El Cajon/Chula Vista) seeks a full-time Physical Therapist to design and implement a comprehensive PT program, promoting independence and safe living in the community. You will conduct assessments, develop personalized plans with the IDT, and oversee their execution across multiple settings.

The role emphasizes geriatric-focused care, reassessments, and collaboration with the interdisciplinary team to optimize function and quality of life for participants.

Qualifications

• Graduate of a PT curriculum accredited by major US professional bodies.
• At least one-year experience as a PT working with geriatric populations.
• Licensed Physical Therapist in the State of California (CA)

Responsibilities

• Design and implement a physical therapy program to help participants maintain independence and live safely within the community.
• Conduct participant assessments and collaborate with the IDT to develop personalized care plans.
• Oversee the execution of care plans, perform reassessments as needed, and coordinate with the IDT to ensure high quality care.
• Deliver services across Day Center, PACE Center, nursing homes, and in-home environments.
• Assist in ordering, inventory, distribution and maintenance of durable medical equipment.
