Edge Recruitment Solutions is assisting a healthcare organization in San Jose, CA to recruit a Physical Therapist. The role focuses on assessing frail, elderly participants both in facility and home settings, developing care plans in collaboration with an interdisciplinary team, and delivering PT interventions per physician orders.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program and CA PT license or license eligibility.
• At least two years of OTA experience in a clinical setting.
• Ability to treat frail, elderly participants with complex conditions.
• Willingness to supervise Therapy Assistants and provide staff training.

Responsibilities

• Conduct PT assessments in facility and in the home to define care needs, preferences, and goals.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document PT interventions per physician orders and care plans.
• Provide active and passive exercise, gait, and ADL training; modalities as appropriate.
• Supervise Therapy Assistants and provide training for Functional Maintenance Programs.
