Job summary

Center for Elders Independence is seeking a Primary Care Physician to lead medical care for participants in our PACE program. Under the Chief Medical Officer, you will guide the interdisciplinary team and oversee care plans to ensure high-quality, coordinated services for elderly participants.

You will perform assessments, supervise clinicians, order tests, coordinate hospital care, and participate in IDT meetings. Requires MD and CA license; geriatrics experience is preferred.

Qualifications

• M.D. degree with active California license to practice medicine.
• Minimum 1 year experience with frail or elderly population.
• Formal training in Internal Medicine or Family Practice; geriatrics preferred.
• Current DEA certificate and BLS certification required.
• Ability to work in a PACE interdisciplinary team and lead care discussions.

Responsibilities

• Provide comprehensive medical care guided by the PACE benefit.
• Conduct initial, 6-month, annual and periodic assessments.
• Develop and implement participant care plans with the IDT.
• Review and sign care plans every 6 months.
• Supervise CEI nurse practitioners and physician assistants.
• Coordinate with hospitalists and arrange appropriate follow-up.
