Job summary

Necnic Group in Central Valley, CA is seeking a Primary Care Physician (Internal Medicine, Family Practice, or Geriatrics) to join our team caring for elderly participants in a PACE program. You will collaborate with an interdisciplinary team to help seniors live independently in their communities.

Key responsibilities include conducting patient assessments, developing care plans, coordinating care in nursing homes and hospitals, guiding Nurse Practitioners, and rotating night and weekend

Qualifications

• BE/BC in Internal Medicine, Family Practice, or Geriatrics.
• Active California medical license.
• Experience with elderly populations or geriatrics care (PACE/long-term care a plus).

Responsibilities

• Conduct patient assessments and develop care plans with the team.
• Oversee care in nursing homes and hospitals, including admissions and discharges.
• Provide guidance to Nurse Practitioners and team members.
• Participate in rotating night and weekend on-call duties.
