Elderwood at Burlington is seeking a compassionate Occupational Therapist to join our skilled nursing and rehabilitation team. You will evaluate residents, develop individualized therapy programs, and collaborate with a multidisciplinary team to promote independence and safe daily living.

Qualifications

• Graduation from an approved school of occupational therapy.
• State license to practice OT.
• Two years' OT experience preferred.
• Current CPR/BLS credential.
• Strong communication and writing skills.
• Ability to work with geriatric and chronically ill residents.

Responsibilities

• Provide skilled occupational therapy evaluations and treatments in accordance with physician orders.
• Develop and update individualized treatment plans and care goals.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary teams regarding resident progress and discharge planning.
• Participate in resident care planning meetings.
• Support resident and family education initiatives.
• Assist with mentorship and clinical support activities, as needed.
