Elderwood at Amherst is seeking a compassionate Occupational Therapist to join our skilled nursing and rehabilitation team. This OT will evaluate and treat residents recovering from illness or surgery, focusing on ADLs, mobility, safety, cognition, and independence.

You will work closely with PT, ST, Nursing, and providers to achieve meaningful functional outcomes. The position offers a collaborative rehab environment with diverse geriatric cases and a focus on quality outcomes, documentation,

Qualifications

• Graduation from an approved occupational therapy program.
• State license to practice occupational therapy.
• Two years' experience preferred.
• Geriatric and subacute care with a team approach.

Responsabilités

• Provide skilled occupational therapy evaluations and treatments per physician orders.
• Develop and update individualized treatment plans and care goals.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary teams regarding resident progress and discharge planning.
• Participate in resident care planning meetings.
• Support resident and family education initiatives.
