Job summary

Elderwood Skilled Nursing & Rehabilitation is seeking a compassionate Occupational Therapist to join our geriatric rehab team. You will focus on patient-centered therapy, functional recovery, and meaningful daily activities to help residents regain independence.

Qualifications

• Graduation from an approved school of occupational therapy is required.
• State license to practice occupational therapy is required.
• Two years’ experience in occupational therapy preferred.
• Interest in geriatric and subacute populations with a team approach.
• Ability to communicate well verbally and in writing is required.

Responsibilities

• Provide skilled occupational therapy evaluations and treatments per physician orders.
• Develop and update individualized treatment plans and goals.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary teams regarding resident progress and discharge planning.
• Participate in resident care planning meetings and family education initiatives.
