Stellar Health Group is seeking an experienced Occupational Therapist to join our rehabilitation team, delivering high-quality care to residents in a supportive, patient-centered environment.

The OT will collaborate with an interdisciplinary team, evaluate patients, develop individualized treatment plans, educate families, and ensure timely documentation and discharge planning in accordance with Medicare and facility requirements. Massachusetts licensed clinicians only.

Qualifications

• Massachusetts OT license required.
• Ability to work within an interdisciplinary team.
• Strong evaluation and assessment skills.

Responsibilities

• Provide occupational therapy services promoting rehabilitation excellence.
• Evaluate patients according to state and facility requirements.
• Develop and implement individualized treatment plans.
• Collaborate with other disciplines and staff.
• Complete Medicare and payer documentation.
• Participate in discharge planning and family conferences.
• Educate patients and families about rehabilitation processes.
• Record and submit treatment charges timely.
• Promote geriatric rehab program to professionals and community.
