Job summary

Elderwood Skilled Nursing & Rehabilitation is seeking a compassionate Occupational Therapist to join our interdisciplinary team. You will evaluate residents, develop personalized therapy plans focused on ADLs, mobility, safety, and independence, and document progress.

This role offers flexible scheduling, a collaborative rehab environment, and a caseload of geriatric and subacute patients. You will help residents regain independence and improve quality of life through meaningful daily

Qualifications

• Graduation from an approved school of occupational therapy.
• State license to practice occupational therapy.
• Two years’ experience in occupational therapy preferred.
• Current, valid CPR/BLS credential.

Responsibilities

• Provide skilled occupational therapy evaluations and treatments in accordance with physician orders.
• Develop and update individualized treatment plans and care goals.
• Maintain timely and accurate clinical documentation.
• Collaborate with interdisciplinary teams regarding resident progress and discharge planning.
• Participate in resident care planning meetings.
• Support resident and family education initiatives.
• Assist with mentorship and clinical support activities, as needed.

Education

Graduation from an approved school of occupational therapy

State license to practice occupational therapy

Two years’ experience preferred
