Job summary

Elderwood at Hornell Skilled Nursing & Rehabilitation is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team. This on-site role emphasizes quality outcomes, individualized treatment plans, and meaningful resident progress within a collaborative interdisciplinary environment.

You will evaluate and treat residents, develop and update care plans, maintain timely documentation, and collaborate with the care team to support mobility, independence, and patient confidence.

Qualifications

• Graduation from an approved school of physical therapy required.
• State license to practice physical therapy required.
• Two years2 experience in physical therapy preferred.
• Ability to direct others necessary.
• This position requires reading, writing, speaking, and understanding English at an intermediate or higher level.

Responsibilities

• Provide skilled physical therapy evaluations and treatments in accordance with physician orders.
• Develop and update individualized care plans and therapy goals.
• Maintain timely and accurate documentation.
• Collaborate with interdisciplinary clinical teams regarding resident progress and needs.
• Participate in resident care planning meetings.
• Assist with education and training initiatives as needed.
• Support a safe, respectful, resident-centered care environment.
