Elderwood Skilled Nursing & Rehabilitation is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team. You will evaluate and treat residents with diverse conditions, develop individualized plans, and collaborate with an interdisciplinary care team to drive meaningful progress.

Qualifications

• Graduation from an approved school of physical therapy.
• State license to practice physical therapy.
• Two years’ experience in physical therapy preferred.
• Ability to direct others as necessary.
• English language proficiency at intermediate level or higher.

Responsabilités

• Provide skilled physical therapy evaluations and treatments in accordance with physician orders.
• Develop and update individualized care plans and therapy goals.
• Maintain timely and accurate documentation.
• Collaborate with interdisciplinary clinical teams regarding resident progress and needs.
• Participate in resident care planning meetings.
• Assist with education and training initiatives as needed.
• Support a safe, respectful, resident-centered care environment.

Formation
