Job summary

Elderwood Skilled Nursing & Rehabilitation is seeking a compassionate Occupational Therapist to evaluate and treat residents across geriatrics and subacute conditions. You will develop individualized plans focusing on ADLs, mobility, safety, cognition, and independence, while collaborating with PT, ST, Nursing, and physicians to optimize rehabilitation outcomes.

Qualifications

  • Graduation from an approved school of occupational therapy.
  • State license to practice occupational therapy.
  • Two years’ experience preferred.
  • Ability to work with geriatric and subacute populations.

Responsibilities

  • Provide skilled occupational therapy evaluations and treatments in accordance with physician orders.
  • Develop and update individualized treatment plans and care goals.
  • Maintain timely and accurate clinical documentation.
  • Collaborate with interdisciplinary teams regarding resident progress and discharge planning.
  • Participate in resident care planning meetings.
  • Support resident and family education initiatives.
  • Assist with mentorship and clinical support activities, as needed.

As an Occupational Therapist at Elderwood, You Will
  • Evaluate and treat residents recovering from illness, injury, surgery, stroke, orthopedic conditions, and functional decline
  • Develop individualized therapy programs focused on ADLs, mobility, safety, cognition, strength, and independence
  • Work closely with interdisciplinary teams to support successful rehabilitation outcomes
  • Help residents safely return to meaningful daily routines and maximize quality of life
  • Maintain accurate clinical documentation and participate in care planning

  • Excellent verbal and written communication skills required.
  • Current, valid CPR/BLS credential.
  • Sense of responsibility and ability to work well with residents, families, and other staff members required.
  • Qualities of maturity, diplomacy, and ability to work well with others required.
  • Ability to work with geriatric and chronically ill residents, as well as other professionals in a friendly and helpful manner necessary.
  • Ability to perform tasks to established standards of excellence required.
  • Regular interaction with residents, coworkers, visitors, and supervisors; requires intermediate or more advanced English proficiency.
