Job summary

Storybrook Care & Rehabilitation in Fort Collins is seeking a compassionate Occupational Therapist to join our in-house therapy team at a midsize facility in Northern Colorado. The position supports cognitive and ADL therapy for long-term residents and reports to the Director of Rehabilitation Services.

Qualifications

• Active state OT license is required and licensure verified.
• Experience in cognitive and ADL therapy essential.
• Ability to supervise and support assistants and students.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop appropriate plan of care.
• Utilize standardized assessments and evidence-based practice to guide interventions.
• Provide rehabilitative, skilled and medically necessary treatment.
• Administer procedures as part of the rehabilitation plan.
• Document patient progress and adjust treatments per regulations.
• Communicate with care teams and families about progress and plans.
• Prepare discharge summaries per regulatory requirements.
