Responsibilities

• Screening and evaluation of patients with physical functioning disorders and wound care, developing care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Administer procedures as part of the rehabilitation plan, including manual techniques, ambulation, and wound care.
• Provide consultation to patients, families, and other service providers.
• Generate discharge summaries compliant with regulatory requirements.
