Job summary

Storybrook Care & Rehabilitation in Northern Colorado seeks a compassionate Physical Therapist to join our in-house therapy team. This full-time role offers a supportive environment with a dedicated clinical team and a pathway to supervise assistants.

You will develop personalized treatment plans, lead patients through exercises, and collaborate with an experienced IDT while providing quality care to long-term residents.

Qualifications

• Active state licensure required.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms; review medical history and referrals.
• Diagnose movement dysfunction by observation and patient history.
• Develop individualized treatment plan and supervise therapy sessions.
• Coach patients and teach therapeutic exercises to improve mobility and reduce pain.
• Maintain accurate patient records and document progress in the electronic system.
