LIFE-NWPA is seeking a Nurse Practitioner to assess, manage, and coordinate healthcare for chronically ill geriatric participants within the One Senior Care family. You will collaborate with physicians and a multidisciplinary team to deliver precise, high-quality care in a structured environment.

Qualifications

Licensed as a Certified Nurse Practitioner in the relevant state.
Experience with geriatric populations preferred.
Strong clinical assessment and care coordination skills.
Ability to work with interdisciplinary teams and physicians.

Responsibilities

Assess and manage health needs of chronically ill geriatric participants.
Coordinate care with physicians and clinical operations.
Document, schedule, and chart patient information accurately.
Identify urgent conditions and initiate appropriate interventions.
Contribute to quality improvement and care planning initiatives.
Provide clinical leadership and education to staff and participants.
