Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to provide collaborative primary care for a panel of senior members in long-term care. You will work with a dedicated clinical leadership team to develop individualized care plans and participate in care team meetings.

You will deliver ISNP Model of Care activities, coordinate with families, PCPs, and nursing facility staff, and ensure care aligns with members’ goals. A strong license, Medicare/NPI, and DEA are required.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• MSN NP from accredited program OR Master’s in Nursing from accredited program.
• Experience in nursing home environment strongly desired.
• Computer/Technology proficiency required.
• Ability to engage and communicate effectively with physicians, members, and family members.
• Self-directed but functions well in a team environment.
• Excellent clinical skills—assessment, diagnosis, treatment, prescribing specifically for the chronically ill population.
• Must have an active national provider number (NPI) and an active Medicare provider number.
• DEA License required.
• Must be eligible to participate in the Medicare program as a provider and registered.

Responsibilities

• Provides collaborative primary care and services to a panel of members.
• Delivers ISNP Model of Care activities and develops advanced care planning.
• Facilitates and administers health plan clinical operations functions—prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure.
• Serves as a trusted advisor to members, loved ones and nursing home staff and colleagues.
• Delivers effective and frequent communication with members’ PCPs, families, and interdisciplinary care team members.
• Supports overall market goals and balances member caseloads.
• Promotes Longevity Health with members, loved ones and nursing facilities and other stakeholders.
• Is able to function autonomously and is accountable for practice.
• Establishes meaningful relationships in the nursing home environment.
• Is member-centric and customer service oriented, establishing meaningful relationships that ensure high-quality care for Longevity Health members.
• Performs other related duties as assigned
