Job summary

Longevity Health seeks a Nurse Practitioner or Physician Assistant to manage a panel of nursing home residents in long‑term care settings. You will conduct comprehensive assessments, develop individualized care plans, and participate in care-team meetings to align with member goals and needs.

You will collaborate with families and physicians, deliver ISNP activities, and drive care coordination, while maintaining autonomy and accountability in a nursing home environment. Location is Columbus, GA.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant
• Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program.
• Experience working in the nursing home environment is strongly desired
• Computer/Technology proficiency required
• The ability to engage and communicate effectively with physicians, members, and family members
• Self-directed but functions well in a team environment
• Excellent clinical skills—assessment, diagnosis, treatment, prescribing for chronically ill population
• Demonstrated critical thinking, organization, and prioritization skills

Responsibilities

• Provides collaborative primary care to a panel of members
• Delivers ISNP Model of Care activities and advanced care planning
• Manages/clinches prior authorization, Part A and B services, medication reviews
• Promotes quality gap closure (HEDIS and STARs) and membership growth
• Serves as a trusted advisor to members, families, and staff
• Communicates with members' PCPs and care team about care plans and changes
• Maintains appropriate patient flow and caseload balance
• Engages with nursing facilities and stakeholders to promote Longevity Health
