Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of nursing home residents, providing comprehensive primary care and developing individualized care plans.

You will work with a dedicated clinical leadership team, conduct assessments, and coordinate with families and an interdisciplinary care team to meet members’ goals and ensure high-quality care.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• MSN for NP or equivalent master’s degree for PA with board certification.
• Experience in nursing home or long-term care settings preferred.

Responsibilities

• Provides collaborative primary care to a panel of enrolled members.
• Develops and implements advanced care planning to reduce hospitalizations.
• Coordinates with the care team, families, and PCPs to align goals and care plans.
