Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to provide primary care for a panel of seniors in long-term care settings. You will perform comprehensive assessments, develop individualized care plans, and collaborate with an interdisciplinary care team to optimize residents' quality of life.

The role requires strong clinical skills, effective communication with families, and the ability to work autonomously within a supportive leadership framework.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• MS in Nursing as NP or Master's degree from a nationally accredited program.
• Experience in nursing home environments is strongly desired.
• Computer/Technology proficiency.
• Ability to engage and communicate effectively with physicians, members, and family.
• Self-directed but functions well in a team environment.
• Excellent clinical skills - assessment, diagnosis, treatment, prescribing for chronically ill.
• Demonstrated critical thinking, organization, and prioritization.

Responsibilities

• Provides collaborative primary care and services to a panel of members.
• Delivers ISNP Model of Care activities and manages care planning to avoid unnecessary hospitalizations.
• Facilitates health plan clinical operations: prior authorizations, Part A/B services, medication reviews, quality gap closure.
• Serves as trusted advisor to members, families, and staff.
• Communicates with members' PCPs and care team about goals of care and changes.
• Manages member caseloads and supports market goals on service delivery.
• Promotes Longevity Health with stakeholders and partners.
• Functions autonomously and is accountable for practice.
