Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to join our healthcare team, delivering high-quality primary care in skilled nursing and senior living facilities. You will provide direct patient care, focusing on wellness, chronic disease management, and care coordination with other providers.

The role requires delivering comprehensive assessments, developing treatment plans, and ensuring smooth operations across multiple facilities.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Provide direct patient care and assessments for residents in SNF and senior living facilities.
• Conduct routine rounds and adjust treatment plans accordingly.
• Coordinate care with facility staff, family members, and other healthcare providers.
• Educate residents and families on disease prevention and wellness.
• Document thoroughly in EMR and ensure regulatory compliance.
• Participate in quality improvement initiatives to enhance care.
