Job summary

Qualifications

• Master's degree in nursing or equivalent required.
• State certification as adult/geriatric nurse practitioner preferred.
• Minimum 3 years’ experience in geriatrics; long-term care experience helpful.

Responsibilities

• Assess patients to identify needs for services and support (DME, meds, HCBS, primary care, hospice).
• Provide primary care with focus on assessment, health management, and prevention.
• Educate patients and families on preventive care, disease management, and adherence.
• Collaborate with Care Coordination team to improve care quality and efficiency.
• Document medical records timely in EMR and ensure accurate coding.
