Job summary

Midland Care Connection is seeking a Nurse Practitioner to deliver patient-centered primary care for older adults across residential and community-based settings. You will assess, diagnose, and manage conditions, coordinating with hospice, home health, and rehabilitation teams to improve comfort and quality of life.

This role emphasizes geriatric care, care coordination, and collaborative practice within an interdisciplinary team.

Qualifications

• Graduate of an accredited Nurse Practitioner program and current RN/APRN licensure.
• Prescriptive authority in Kansas is required or in-state eligibility.
• Experience in primary care, geriatrics, or chronic disease management is preferred.
• Strong assessment, communication, and care coordination skills.
• Ability to work collaboratively within an interdisciplinary healthcare team.

Responsibilities

• Performing comprehensive physical assessments of older adults and other patients.
• Ordering and interpreting diagnostic tests as part of ongoing care.
• Managing medications and reviewing treatment plans with patients and families.
• Addressing acute illnesses and chronic disease management in various settings.
• Providing dementia and geriatric-focused care and counseling to families.
• Collaborating with physicians, nurses, caregivers, hospice, and home health teams to coordinate care.
