Job summary

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to provide primary care to a panel of nursing home residents. You will work with our leadership team to implement advanced care planning and a treat-in-plan model aimed at reducing hospitalizations.

You will coordinate with members, families and a multidisciplinary care team, ensuring goals and plans align with member needs. The role requires clinical excellence and autonomous accountability in a geriatric setting.

Qualifications

• 6+ months experience as a Nurse Practitioner or Physician Assistant.
• Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program.
• Experience in nursing home environment is strongly desired.
• Strong communication with physicians, members and families.

Responsibilities

• Provides collaborative primary care to a panel of members.
• Delivers ISNP Model of Care activities and advanced care planning.
• Manages prior authorization, services, medication reviews, quality gap closure.
• Acts as trusted advisor to members, families and staff.
• Communicates with members’ PCPs and care team about needs and plans.
• Maintains autonomy and accountability in the nursing home setting.
