CareMed seeks a Nurse Practitioner to deliver primary and preventive care for residents in long-term care facilities. You will manage acute changes, engage in advanced and chronic care planning, and collaborate with physicians to ensure optimal patient outcomes.

Strong EMR skills and experience in geriatric care are essential. Qualifying candidates hold OH NP licensure, board certification, and DEA registration.

Qualifications

• Current, unrestricted NP license in the state of OH.
• Board Certification in a relevant NP specialty.
• DEA licensure to prescribe controlled substances as allowed by law.
• Clinical background in adult, family or geriatric care.
• Experience in long-term care settings with EMR systems.
• Ability to manage patients with advanced illness and chronic conditions.
• Ability to work in an interdisciplinary care team.

Responsabilités

• Provide primary and preventive care for long-term care residents.
• Manage acute changes in condition and coordinate treatments.
• Engage in advanced care planning and chronic care management.
• Prescribe medications in accordance with state law.
• Order and follow up on diagnostic tests and treatments.
• Communicate with nursing home staff regarding patient changes.
