Job summary

CareConnectMD seeks an experienced Adult-Gerontology Nurse Practitioner to assess and manage care for frail and elderly patients in a long-term care setting. You will coordinate services, educate patients and families, and collaborate with the care team to optimize outcomes.

Role emphasizes health management, prevention, and person-centered planning across multiple locations; strong EMR and communication skills are essential.

Qualifications

• Master’s degree in nursing or equivalent required.
• State certification as adult/geriatric nurse practitioner required.
• Minimum 3 years’ geriatrics experience; long-term care/ skilled nursing facility experience preferred.

Responsibilities

• Assess patients to identify needs for services and support (DME, medical supplies, HCBS, hospice, etc.).
• Provide primary care in a long-term care setting with emphasis on assessment, health management, education, and prevention.
• Collaborate with Care Coordination team; document care in EMR; communicate with PCPs and families.
