Job summary

Qualifications

• Master's degree from accredited Nursing school.

Responsibilities

• Assess patients to identify needs for services such as DME, HCBS, primary care, and specialty care.
• Assist patients and families in goals of care, environmental and linguistic needs for the care plan.
• Manage a complex caseload of patients.
• Provide primary care in long-term care settings with focus on assessment, health management, education, and prevention.
• Educate patients and families about preventive care, disease management and medication adherence.
