Qualifications

• Current California Licensed Registered Nurse, plus one year of clinical nursing experience or one year of case management with frail older adults.
• Ability to organize time and set priorities to certify Level of Care determinations and implement services.

Responsibilities

• Conducts in-depth assessments and reassessments covering medical, health, and rehabilitation concerns within deadlines.
• Certifies level of care determinations.
• Develops and implements individualized care plans with the SWCM.
• Monitors needs and provisions in the care plan.
• Collaborates with physicians and other health professionals.
• Provides health education and staff in-service training.
