Job summary

You will represent the program in the Elder Abuse Consortium and engage with families and service providers in the community.

Qualifications

• Bachelor's degree or equivalent in health, social work, counseling, psychology, gerontology or related field.
• Case management experience may substitute for education.
• Ability to communicate effectively with consumers and staff of other service agencies.
• Sensitive to the needs of the elderly and disabled from diverse cultures.
• Bilingual and bi-literate preferred (English, Chinese, Spanish, Vietnamese, etc.).
• Must travel using public transport or a vehicle with valid CA license and insurance.

Responsibilities

• Provide case management services to seniors and adults with disabilities.
• Conduct intake and comprehensive home assessments.
• Develop, implement, and monitor care plans.
• Coordinate and arrange services; maintain client files and reports.
• Report incidents to appropriate authorities and participate in program meetings.
• Engage in outreach and education on elder abuse within the community.
