Qualifications

• Bachelor's degree in social work or related field plus two years of experience working with frail older adults.
• Ability to assess the consumer’s need for services and perform the core functions of case management; elder abuse training preferred.
• Strong communication skills with consumers, families, and staff; ability to work in a multidisciplinary team.
• Bilingual and bi-literate preferred (English, Chinese, Spanish, Vietnamese, Tagalog, etc.).
• Must travel via public transportation or personal vehicle; CA driver license and insurance required.

Responsabilités

• Provides case management services to seniors.
• Intake and comprehensive home assessment of the consumer’s functional and cognitive capacity.
• Collaborate with the Supervising Care Manager in developing the individualized care plan.
• Arrange, coordinate, and verify services identified in the care plan.
• Follow up and monitor the plan to ensure it meets the consumer’s needs.
• Maintain complete client files and required data collection forms and reports.
• Report incidents affecting case management clients to appropriate bodies.
• Conduct outreach and education to seniors, families, and service providers about elder abuse.
• Participate in training, meetings, and quality assurance activities.
