myPlace Health is seeking a Nurse Practitioner to provide comprehensive clinical care for elderly participants across center, home, virtual, and SNF settings. You will lead care planning, manage medication orders, and collaborate with interdisciplinary teams to ensure wraparound, whole-person care.

A California NP license and prescriptive authority are required, with an emphasis on geriatrics. The role offers onsite work in California, competitive salary, and a full benefits package, including

Qualifikationen

• Two years of NP practice in medically complex or geriatric settings.
• Active, unrestricted California NP license and CA RN license where applicable.
• Master’s in Nursing and NP program from an accredited school.
• Prescriptive authority as NP, current CPR certification.
• Commitment to geriatrics and chronic care management.

Aufgaben

• Be a core clinical voice on the IDT, contributing to care planning and service determination requests.
• Own and grow your participant panel with continuity and accountability.
• Prescribe, evaluate, and update medication and treatment orders with required approvals.
• Coordinate day-to-day care in real time to keep participants stable and progressing toward goals.
• Document interactions and next steps in the EMR within center timeframes.
• Refer to specialists and ensure follow-through with approvals.
• Conduct histories and physicals and support informed decision-making.
• Order, interpret, and follow up on labs and results; close loops promptly.
• Perform selected procedures within scope while maintaining high-quality care.
• Identify abnormalities and elevate concerns to ensure participant safety.
• Collaborate with behavioral health, pharmacy, and other care partners for wraparound care.
• Meet participants where they are—center, home, virtual, SNFs, and other settings.
• Support meaningful goals-of-care conversations with empathy and clarity.
• Capture acuity accurately with HCC to reflect clinical complexity.
• Provide on-call coverage to ensure timely guidance.
