Job summary

CareSource is seeking a Nurse Care Manager to provide comprehensive care coordination, including in-home assessment, planning, advocacy and authorization of services to meet members’ health needs. You will work with members, families, PCPs and the interdisciplinary team to promote quality, cost-effective outcomes.

The role requires an Associate Degree in Nursing with RN licensure in New York; a Bachelor’s in Nursing is preferred and 2+ years of RN experience, preferably in geriatrics or managed

Qualifications

• Associates degree in Nursing required.
• Two years of RN experience required.
• Geriatrics or managed long-term care experience preferred.
• NY licensure required; CMSA standards knowledge a plus.

Responsibilities

• Provides care coordination including in-home assessment and planning.
• Authorizes covered services and coordinates care for members.
• Communicates with member, family, primary care practitioners and IDT.
• Conducts home visits to assess living situations and needs.
• Develops Care Plans and Patient Centered Service Plans with IDT.
• Ensures safe discharge from the plan and documents per policy.
• Maintains monthly recertifications and continual care management.
