Job summary

Self-Help For The Elderly in San Francisco, CA is seeking a Care Manager Aide to support the MSSP Team in the care management process under the supervision of the Care Manager. This Regular Full-Time role helps frail seniors remain independent and avoid premature nursing facility placement.

Key duties include intake screening, completing applications, providing information, and coordinating with the primary care manager.

Qualifications

• Two years of experience working with frail older adults or a Bachelor's degree in a human services discipline.
• Knowledge, skills, and experience necessary to assess the consumer’s need for services and perform the core functions of case management.
• Ability to communicate effectively with consumers or their representatives and staff of other service agencies.
• Sensitive to the needs of the elderly and disabled adults from diverse cultures.

Responsibilities

• Provides case management services to seniors. Conducting intake/Screen on MSSP applicants.
• Completing the intake and application process.
• Providing information and assistance to the participant as needed.
• Purchasing and delivering items requested by the primary care manager.
• Completing monthly care management contacts.
• Completing quarterly home visits.
• Documenting activities in the participant record.
