Job summary

The Institute on Aging in San Mateo, CA, seeks a Care Manager II to conduct comprehensive assessments and manage individualized Community Living Plans for clients with complex needs.

Qualifications

• MSW or related master's degree; or BA/BS in Social Work with 2+ years of social-work experience.

Responsibilities

• Conduct comprehensive assessments and ongoing re-assessments of clients including psychosocial, physical, mental health, environmental and spiritual needs.
• Write detailed assessment reports and initiate client-centered Community Living Plans.
• Perform home, hospital, and facility visits; escort clients to medical appointments as needed.
• Coordinate placement and monitor services with Medicare, Medi-Cal and other programs.
• Maintain case notes, documentation and charting for continuity of care.
• Educate clients and support networks about available resources and services.
• Ensure ethical and legal standards are followed and participate in CQI activities.
