Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to improve outcomes for residents in long-term care settings. You will collaborate with a team of clinicians and care managers to deliver patient-centered care and coordinate services within Medicare Advantage plans.

You will exercise independent judgment, treat acute and chronic conditions, and work closely with facility partners to implement our national model of care in compliance with regulations.

Qualifications

• Masters or better degree in nursing or related field
• APRN/PA license required (FNP/AGNP/AHACNP or PA)
• 3 years’ experience in clinical nursing or rehab in geriatric populations
• Electronic Health Records experience
• MS Office (Word, Outlook, Excel) proficiency

Responsibilities

• Assess patients’ medical and mental health needs and develop a plan of care for proactive management
• Work with RN case managers as part of an integrated care team
• Prescribe medications and order labs, diagnostics and consultations
• Monitor patient response and adjust treatment plans accordingly
• Ensure compliance with local, state, and federal regulations related to clinical services
