Job summary

TruHealth is seeking an advanced practice provider (NP or PA) to deliver primary care for seniors in long-term care settings. You will work with a team of providers and case managers to improve outcomes for residents enrolled in Medicare Advantage plans.

You will exercise independent judgment for acute and chronic care, with supervision as appropriate, focusing on quality of life and prevention of hospital visits.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience; MS Word/Outlook/Excel familiarity.

Responsibilities

• Assess patients’ medical and mental health needs and develop a care plan to proactively manage conditions.
• Ensure compliance with local, state, and federal regulations for clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor patient compliance and response to treatment; adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
