Job summary

TruHealth is seeking an advanced practice provider, either an NP or PA, with experience in primary care or internal medicine and a geriatrics focus. You will work within nursing homes or assisted living communities to deliver personalized, coordinated health care that improves quality of life and reduces hospital visits.

You will exercise independent judgement, collaborate with supervising physicians, and implement the national care model in compliance with regulations, supporting our residents

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Office (Word, Outlook, Excel).

Responsibilities

• Assessing medical needs and providing a plan of care for geriatric residents.
• Ensuring compliance with local, state, and federal regulations related to clinical services.
• Prescribing medications and ordering labs, diagnostics and consultations.
• Monitoring treatment response and adjusting plans as needed.
• Collaborating with RN case managers as part of an integrated care team.
