Job summary

TruHealth, part of American Health Partners, seeks an advanced practice provider (NP/PA) to deliver primary care for seniors in long-term care settings. You will work with a multidisciplinary team to improve quality of life and reduce hospital visits.

Candidates should be licensed APRN/NP or PA with geriatrics experience, comfortable with independent judgment and coordination with supervising physicians. On-site nursing home environments are typical.

Qualifications

• APRN/NP or PA license required.
• 3 years’ experience in geriatric care/rehab.
• EHR experience and MS Office proficiency.

Responsibilities

• Assess patients’ medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs and consultations.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers in an integrated care team.
