Job summary

TruHealth, part of American Health Partners, seeks an advanced practice provider (APRN/NP or PA) with geriatrics focus to improve outcomes for long-term care residents. You will collaborate with a multi-disciplinary team to deliver coordinated healthcare that enhances quality of life and reduces hospital visits.

Responsibilities include patient assessments, medication management, order of labs and diagnostics, and adherence to federal/state regulations, within nursing homes and assisted living

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care that proactively manages their conditions and addresses barriers to care.
• Ensure compliance with local, state, and federal agencies related to clinical services you provide.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients’ compliance and response to their treatment and modify plans accordingly.
• Work with RN case managers as part of an integrated care team.
