Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to improve outcomes for long-term care residents. You'll collaborate with a team of providers and case managers to deliver proactive, coordinated care in nursing homes and assisted living communities.

You will work with APRN or PA licensure, 3 years in geriatric clinical nursing, and EHR experience, ensuring compliance with regulations while offering a competitive compensation and benefits package.

Qualifications

• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• APRN/PA license required.
• ED or hospital-based geriatrics exposure is a plus.

Responsibilities

• Assess and manage residents’ medical and mental health needs.
• Coordinate care with RNs and case managers.
• Prescribe medications and order labs/consultations.
• Ensure compliance with state/federal regulations.
• Provide care within nursing homes or assisted living settings.
