Job summary

TruHealth in the United States is seeking an advanced practice provider—NP or PA—with a geriatrics focus to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers to deliver coordinated care within nursing facilities enrolled in Medicare Advantage plans.

You will work with supervising physicians as needed, applying independent judgment to treat acute and chronic conditions while adhering to federal and state regulations.

Qualifications

• FNP, AGNP, AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications, including Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order lab work and consultations.
• Monitor patient progress and adjust treatment plans.
• Collaborate with RN case managers as part of an integrated care team.
