Job summary

TruHealth seeks an advanced practice provider (NP/PA) to join our geriatrics-focused team serving residents in nursing homes and assisted living communities. You will deliver coordinated, personalized care under a national care model, improving outcomes and quality of life for seniors on Medicare Advantage plans.

APRN/NP or PA license is required with a graduate degree from an accredited program and 3 years in geriatric clinical nursing or rehab.

Qualifications

• APRN/NP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft applications (Word, Outlook, Excel).

Responsibilities

• Assess patients’ medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal agencies related to clinical services.
• Prescribe medications and order lab work, diagnostics and consultations.
• Monitor patients’ response to treatment and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
