TruHealth is hiring an advanced practice provider (APRN/NP or PA) to deliver primary care to seniors in long-term care settings. You will work with a team of providers and case managers to improve outcomes for residents enrolled in Medicare Advantage plans.

The role offers a strong benefit/pay package and clear opportunities for growth. Applicants should have geriatrics experience, an APRN or PA license, and EHR skills.

Qualifikationen

• FNP, AGNP, AHACNP or PA license required
• Degree from an accredited APRN or PA program
• 3 years’ experience in clinical nursing or rehab in geriatric populations
• Electronic Health Records experience
• Working knowledge of Microsoft applications, including Word, Outlook and Excel

Aufgaben

• Assess patients and plan proactive care addressing medical/mental health needs
• Ensure compliance with local, state, and federal regulations for clinical services
• Prescribe medications and order labs, imaging, and consultations
• Monitor treatment response and adjust plans as needed
• Collaborate with RN case managers as part of an integrated care team
