TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA), with experience in primary care or internal medicine and a geriatrics focus. You will work with a team of providers and case managers in nursing homes or assisted living communities enrolled in Medicare Advantage plans to improve resident outcomes.

Qualificações

• APRN/PA license required.
• 3 years’ clinical nursing or rehab in geriatric populations.
• Masters or better required.
• Experience with Electronic Health Records.

Responsabilidades

• Assess patients’ medical and mental health needs and develop a care plan.
• Prescribe medications and order labs, procedures, and consultations.
• Coordinate care with RN case managers and facility staff.
• Ensure compliance with federal, state, and local regulations.

FNP/AGNP/AHACNP PA license
