Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers, offering a market-competitive pay package and growth opportunities.

Our APRN/PA role focuses on personalized, coordinated care for seniors enrolled in Medicare Advantage plans, with independent judgment to treat acute and chronic conditions in nursing homes or assisted living facilities.

Qualifications

• FNP/AGNP/AHACNP or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients’ medical and mental health needs and develop a proactive plan of care.
• Ensure compliance with local, state, and federal regulations governing clinical services.
• Prescribe medications and order labs, diagnostic tests and consultations.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
