TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to join our team serving long-term care residents. You’ll collaborate with providers and case managers to improve patient outcomes and quality of life in nursing homes or assisted living communities.

You will exercise independent judgment with supervision as needed, prescribe medications, order labs, and participate in our national model of care while complying with federal and state regulations.

Qualifikationen

• FNP, AGNP, AHACNP or PA license required
• Degree from an accredited APRN or PA program
• 3 years’ experience in clinical nursing or rehab in geriatric populations
• Electronic Health Records experience
• Working knowledge of Microsoft applications, including Word, Outlook and Excel

Aufgaben

• Assess patients’ medical and mental health needs and create a proactive plan of care
• Ensure compliance with local, state, and federal agencies in clinical services
• Prescribe medications and order labs, diagnostics, and consultations
• Monitor patient progress and modify treatment plans as needed
• Collaborate with RN case managers as part of an integrated care team
